Income-Driven Repayment Application Tutorial

You’ve selected Married, but Separated. You’ll want to complete the following sections.

SECTION 1: BORROWER INFORMATION

Fill in the area indicated below with your current information.

SECTION 2: REPAYMENT PLAN OR RECERTIFICATION REQUEST
  1. Check the first box to begin an income-driven plan.
  2. Check the first box to select the plan with the lowest monthly payment.
  3. Do you have multiple loan servicers? Neither choice will impact your application, but you’ll want to send a separate application to each servicer.
  4. As part of loan rehab, you are not in deferment or forbearance. Check the first box.

SECTION 3: FAMILY SIZE INFORMATION

Fill in the two areas indicated below with your current information.

Be sure to include your name and Social Security number where indicated on the following pages.

What is your marital status? Check “Married, but separated” and skip to question 11.

You do not need to fill in section 4c under your situation.

You will need to include a copy of your most recent tax return with this application, unless you haven’t filed or your income has changed significantly. In that case, see section 5.

SECTION 5: FOR BORROWERS WITH TAXABLE INCOME*

For those not sending a tax return due to a change in income, here is where you will find the list of acceptable documents to prove your income.

Gather the appropriate documentation and move to Section 6. *If you don’t have taxable income, move to Section 6.

SECTION 6: BORROWER REQUESTS, UNDERSTANDINGS, AUTHORIZATIONS, AND CERTIFICATION

Fill in your name and Social Security number.

DO NOT check the box requesting a one-month forbearance; Trellis borrowers just coming out of the loan rehabilitation program are not yet eligible.

After signing and dating the form, mail it along with any required proof of income documents to your new servicer.
